NHS England would like to link information from all their resources together to support the health, treatment and care you receive.
To do this they are asking if they can take information from your records. The information such as your postcode, date of birth, NHS number (but not your name) will be linked to your hospital record and will be used by researchers and those planning health services to make sure the best care possible can be provided for everyone.
You have a choice!
You have the right to prevent confidential information about you from being shared or used for any purpose other than providing your care, except in special circumstances. If you do not want information that identifies you to be shared outside your GP practice-other than where necessary by law (eg if there is a public health emergency), call into the surgery and ask Reception for a form to complete.
